SB003: Maagzuur / Stomach acid - advanced (Nederlands/ English after the break)

Dan nu voor het tweede deel van dit miniprojectje. Iets meer diepgang in hoe maagzuur en vertering wordt gereguleerd.



Even kort herhalen:
- Eiwitten worden in de maag afgebroken door het enzym pepsine tot kleinere stukjes, genaamd peptones.
- Pepsine ontstaat uit pepsinogeen, dat wordt gemaakt door chief cells in de maagwand.
- Maagzuur, dat wordt gemaakt door parietal cells in de maagwand, verandert pepsinogeen in pepsine en zorgt voor een zure omgeving waarin pepsine goed werkt.
- De maagwand wordt beschermd tegen het maagzuur en de pepsine door mucous cells, die een basische laag slijm (mucus) maken. Het basische milieu neutraliseert het zuur en inactiveert de pepsine.

Een belangrijk concept in het lichaam is het aan- en uitzetten van verschillende processen om een goed biologisch evenwicht (homeostase) te behouden. Het lichaam is constant aan het veranderen en er moet dan ook op tijd een compensatie zijn voordat de optimale balans verloren gaat. Het proces van constante compensaties om een optimale lichamelijke conditie te behouden, wordt homeostase genoemd. Het zijn voornamelijk de hersenen en het zenuwstelsel die alle relevante factoren in de gaten houden en het lichaam aansturen als compensatie nodig is. Dit gebeurt volledig automatisch, zolang het lichaam gezond is. 

De aanknop van de maag is het zenuwstelsel. Zodra je iets ziet om te eten, ruikt om te eten, proeft, kauwt en doorslikt, begint de maag zich al klaar te maken voor vertering. Het autonome zenuwstelsel stuurt namelijk een algemeen signaal dat de vertering moet beginnen, omdat er is/wordt/zal worden gegeten. De maag begint daardoor met het maken van zuur en pepsinogeen, en extra mucus ter bescherming.
Als het eten in de maag terecht is gekomen, is dat weer een extra signaal om maagsappen te maken. Het voedsel rekt de maag wat uit en als reactie wordt er meer zuur gemaakt.


ECL cells (EnteroChromaffin-Like cells) liggen in de maagwand onder het epitheel (dus zijn niet direct in contact met de inhoud van de maag) dicht bij de parietal cells. ECL cells maken histamine, dat ze vervolgens om zich heen loslaten. De parietal cells maken meer maagzuur als ze de histamine opmerken. 
De ECL cells zijn endocriene cellen, omdat zij de histamine in (endo) het lichaam afgeven (crien), in tegenstelling tot de parietal en chief cells, die hun producten buiten (exo) het lichaam afgeven (crien). Technisch gezien wordt de binnenkant van het verteringskanaal als 'buiten het lichaam' beschouwd, omdat het in direct contact staat met de buitenwereld zonder dat er een laag cellen tussen zit. Het verteringskanaal is grofweg een lange buis, geheel bedekt met epitheel. Alles dat zich achter het epitheel bevindt, wordt als intern milieu beschouwd (zo ongeveer).

Histamine komt misschien bekend voor in een andere context. Het is namelijk een belangrijke signaalstof voor het immuunsysteem en is hoofdzakelijk de oorzaak van de symptomen van een allergische reactie. Over het algemeen verhoogt  histamine de ontstekingsreactie, maar hier wordt het dus gebruitk in een andere context.
Kort samengevat: de ECL cell maakt histamine, wat de parietal cell aanzet tot het maken van meer zuur. Dus ECL cells verhogen de zuurtegraad van de maaginhoud.


De volgende laag van complexiteit komt vanuit de bodem van de maagwandcrypt, waar de maag endocriene cellen zich bevinden. Een soort daarvan, de G cell, maakt gastrin en verspreidt dit in de directe omgeving. Gastrin bevordert (of stimuleert) de histamine productie van ECL cells en de zuurproductie van parietal cells. Dit verlaagt de pH van de maag dus nog meer.


Als de vertering van eiwit een beetje op gang is gekomen, begint een positive feedback loop: het resultaat van het proces versterkt het proces. In dit geval werken de peptones (kleine stukjes eiwit) als signaal voor G cells en chief cells om nog harder te werken. G cells maken daardoor meer gastrin, waardoor ECL cells meer histamine maken en parietal cells meer zuur. Peptones stimuleren chief cells  om meer pepsinogeen te maken, dat in de zure omgeving pepsine wordt, waardoor nog meer eiwit wordt afgebroken. Meer afgebroken eiwit geeft ook meer peptones, dus het signaal versterkt zichzelf. Natuurlijk alleen maar zolang de peptones aanwezig blijven. Zodra de maaginhoud voldoende verteerd is, gaat het verder naar de darmen en wordt de positieve feedback verbroken.


Het lichaam vereist regulatie op meerdere niveaus en in de maag komt dat in de vorm van de tweede type endocriene cel op de bodem van de crypt. D cells, via de productie van somatostatin (Grieks voor: zet het lichaam stil (ongeveer)), zijn de rem op maagzuur productie. De D cell doet aan het begin van de vertering in de maag nog niet zo veel. Maar wanneer de pH van de maag lager wordt en de hoeveelheid gastrin in de omgeving hoger, dan begin de afgifte van somatostatin aan de omgeving. Somatostatin remt de processen van ECL, G en parietal cells en remt daarbij de productie van maagzuur. Hoe sterker de signalen zijn voor het produceren van maagzuur, hoe meer D cells worden gestimuleerd, wat dus als tegenreactie leidt tot de afremming van maagzuurproductie door somatostatin. Dit is een typische negatieve feedback loop: het resultaat van een proces remt het proces.
Somatostatin wordt ook gemaakt door cellen in het begin van de darm (duodenum) als reactie op het legen van de maag. Omdat somatostatin endocrien wordt afgegeven, heeft het effect op meer dan alleen de directe omgeving en kan ook via bloed worden gecirculeerd.

Als laatste: de hele maag staat onder de controle van het zenuwstelsel, voornamelijke het parasympatische deel van het autonome zenuwstelsel, zoals eerder besproken. Dit deel werkt in het algemeen om spijsvertering en rust te bevorderen. De signaalstof die hiervoor wordt gebruikt, is acetylcholine. De zenuwuiteindes laten acetylcholine los in de omgeving van de maag. Acetylcholine bevordert de functie van alle maagcellen hier genoemd, met uitzondering van D cells, die juist worden geremd door acetylcholine. Dat was wel te verwachten, gezien de functie van het parasympatische zenuwstelsel en D cells. 


Dat is zo'n beetje wat ik weet over hoe maagzuurregulatie en eiwitvertering werkt. Dit is volgens mijn studieboek, natuurlijk (immers al 5+ jaar oud). Het boek meldde ook dat er het proces nog niet volledig was begrepen en dat er meerdere ongeidentificeerde cellen ook een effect hebben. En natuurlijk keken we hier alleen naar de maag en niet naar de rest van het lichaam. Ook ging ik er voor de eenvoud vanuit de elke cel maar 1 ding deed, wat niet het geval zal zijn.

Ik hoop dat het interessant was zo. Ik vind het zelf altijd leuk om de complexiteit te vinden achter op het oppervlak eenvoudig ogende processen. In mijn ervaring is niets zo eenvoudig of vanzelfsprekend als dat het lijkt.



And now to continue with the second part of this mini project. This time with slightly more in-depth discussion regarding stomach acid regulations and protein digestion.


A brief summary:
- Inside the stomach, protein is broken down by the enzyme pepsin into smaller pieces, called peptones.
- Pepsin is derived from pepsinogen, which is made by chief cells in the stomach lining.
- Stomach acid made by parietal cells in the stomach lining transforms pepsinogen into pepsin and creates an acid environment to optimize the function of pepsin.
- The stomach lining is protected from stomach acid and pepsin by a layer of alkaline mucous made by mucous cells. The alkaline environment neutralizes the acid and inactivates the pepsin.

I will continue to discuss what other cell types and systems influence this proces. An important concept when studying the body is its ability to selectively activate or inactivate processes involved in maintaining a physiological balance (homeostasis). The body changes constantly and it is therefore required to compensate for outside (and inside) influences to maintain homeostasis. Homeostasis means maintaining an internal environment that is optimized for the body at that time. To this end, most critical processes are regulated to stay within tight boundaries. As the environment changes, however, these boundaries will change accordingly as well. And as the balance for any one factor falls outside of these boundaries, the body will act to return to a balanced internal environment. Homeostasis in an unconscious process, managed and monitored by the brain and nervous system.
The on-switch for the stomach is therefore also neurological. As soon as you see something to eat, smell something to eat, taste, chew and swallow, the stomach is made ready to digest. The autonomic nervous system has already sent a general signal to start digestion, since it knows that food will be/is being/has been eaten. The stomach starts producing acid and pepsinogen to initiate digestion, as well as extra mucous to protect itself.
Food entering the stomach provides an additional signal to promote the production of acid and digestive enzymes.


ECL cells (EnteroChromaffin-Like cells) are located close to parietal cells, but behind the layer of epithelial cells in the stomach lining (so they are not in direct contact with the stomach content). ECL cells produce histamine, which they release into their immediate environment. Parietal cells then produce more acid when they notice the presence of histamine.
ECL cells are so-called endocrine cells, because they release (crine) histamine inside (endo) the body, whereas parietal and chief cells release their product on the outside (exo) of the body. Technically, any part of the body that is not behind a layer of epithelium is internal. The digestive tract is technically a single long tube, which is lined by epithelium of some kind and is therefore considered external to the body.
Histamine may be familiar to some in a different context. Histamine is an important messenger for the immune system en is the main cause of allergic symptoms. Generally speaking, histamine signals for an increase in inflammation. But in the stomach it appears to have a different function.
Brief summary: ECL cells produce histamine, which makes parietal cells produce more acid. ECL cells therefore indirectly increase the acidity of the stomach content. 


The next layer of complexity comes from the bottom of the stomach crypts, where the stomach endocrine cells are located. One type of endocrine cell, called G cells, produce gastrin and secrete it in their direct environment. Gastrin stimulates histamine production by ECL cells and acid production by parietal cells, thereby lowering the pH of the stomach indirectly.


Once protein digestion has started, a positive feedback loop kicks in. A positive feedback loop occurs when the result of process enhances the process it resulted from, effectively amplifying it. In this case, peptones (the small protein fragments) stimulate G and chief cells. G cells produce more gastrin, which increases acid production and chief cells produce more pepsinogen. Pepsinogen in the acidic environment becomes pepsin and breaks down more proteins into peptones, which amplifies the peptone signaling. This loop remains active as long as peptones are present. As soon as the stomach content is sufficiently digested, it will move on to the small intestines, thereby breaking the feedback loop.


The body regulates most processes at multiple levels. In the stomach, the second type of endocrine cell, D cells, provide more regulation for stomach acid production. D cells make somatostatin (Greek for: halting the body (or something like that)). D cells do very little when digestion has just gotten started, but start producing somatostatin once the pH gets lower and more gastrin is made by G cells. Somatostatin inhibits (stops) ECL, G and parietal cells from producing their products and thereby reduces the amount of acid that is being made directly and indirectly. Stronger activating signals for D cells result in higher amounts of somatostatin being released. This is known as a negative feedback loop, where the result of a process, inhibits that process.
Somatostatin is also made by cells in the small intestine (duodenum) as response to a lower pH when the stomach content gets enters the small intestine. Endocrine release of somatostatin can be transported by blood to provide signals elsewhere in the body, not just the immediate environment around the endocrine cell.

A final regulation of stomach acid, as mentioned before: the entire stomach is under the control of the nervous system, mainly the parasympathetic branch of the autonomous nervous system. This branch promotes digestion and rest. It used acetylcholine, locally made by nerve endings, to communicate with the stomach. Acetylcholine stimulates cells that produce digestive enzymes and acid, but it inhibits D cells. Given the effect of somatostatin produced by D cells, the inhibition of D cells by acetylcholine in this context, makes sense, since it results in more digestion. Of course, the inhibition of D cells by the nervous system can be overcome with sufficient stimulatory inputs.


That is some of the information I gathered from my book on human physiology regarding stomach acid regulation and protein digestion. My book is at least 5+ years old by now and mentioned that several unknown cells and signals are also suspected to affect the stomach. And, of course, my descriptions are simplified for your understanding ;).
Regardless, I hope you can now appreciate a bit more of the complexity involved in a seemingly simple process. This is a common theme, in my experience, when studying anything in-depth.

    1. Thanks! I love making illustrations like this, but of course I am still making my usual pencil drawings. At the moment, I am not making anything worth putting on my blog though, just some random things I feel like making. Hope to hear from you again soon in a longer format maybe ;)
